Can you request conservative care while still treating active disease?

Published October 20, 2026

In short

Yes. Conservative care means removing only what is diseased, preferring the smallest restoration that will work, choosing materials with you, and preventing rather than repairing. It does not mean leaving active decay, an infected nerve or periodontitis untreated, because those conditions progress and the conservative choice is usually the earliest effective treatment. Preferences shape how and with what; the diagnosis decides whether.

What changes the answer

The stage of disease: an early lesion that can be arrested versus cavitated decay (IQWiG); gingivitis versus periodontitis (NIDCR); reversible versus irreversible pulp inflammation. Urgency signals. Your medical history. Whether a preferred material or technique has evidence for that situation.

If any of these apply, this is not a dental-office problem first: trouble breathing or swallowing, rapidly spreading swelling of the face or neck, fever with facial swelling, bleeding that will not stop, or a serious injury to the head or jaw. Call 911 or go to the nearest emergency department. Do not use the appointment request form for these.

Conservative-but-complete care

SituationConservative and completeNot conservative, just incomplete
Early enamel lesionFluoride, sealing, diet, recheck (FDA)Ignoring it without a recheck
Cavitated decaySelective removal; smallest bonded restorationLeaving soft decay under a "watch"
Deep decay, healthy pulpVital pulp therapy where indicated; sealed restorationHoping it settles
Infected pulpRoot canal treatment or extractionAntibiotics alone
GingivitisCleaning and home care
PeriodontitisScaling and root planing; maintenanceA polish
Cracked toothBonded cusp coverage (AAE)A filling that leaves the crack unsupported
Sound old amalgamLeave it (FDA)Replacing it without a reason

How to say it

"I want the least invasive option that actually treats the problem, and I'd like to choose materials with you." That sentence asks for both halves.

What the dentist should explain

Which findings are active, which can be arrested, what the smallest effective option is for each, and which preference has evidence behind it. The studio describes biologic and biomimetic dentistry as conservative and complete (studio).

Next step: Request a consultation or call (805) 565-5111. A request is not a confirmed appointment; the office confirms times during its listed hours (Monday to Thursday, 8 am to 5 pm, plus select Fridays).
