How a dentist evaluates an older crown
Age alone does not condemn a crown. An old crown that seals the tooth, has a healthy margin and a healthy nerve underneath is usually monitored, not replaced. The exam, not the calendar, decides.
What the dentist checks, in order
- Your history: sensitivity, food catching, a changed feel, bad taste
- The margin, with an instrument and mirror: open edges, roughness, staining versus decay
- Gum health and recession exposing root or an old edge
- Mobility or looseness of the crown
- Bite contacts and wear through the material
- Chips or fractures
- X-rays, taken when the clinical findings call for them; the American Dental Association's selection criteria base frequency on individual risk rather than a fixed interval (ADA)
Why crowns are replaced when they are
Decay at the margin, loss of the cement seal, fracture or chipping, nerve problems in the tooth under the crown, and appearance. Published cohorts of single crowns show failures accumulating over years for these reasons rather than at a fixed expiry; in one multifactor study, material type was not the main predictor. The Oral Health Foundation notes crowns can last many years with care (OHF).
What "watch" means
Documented findings, photographs, a note to recheck at the next visit, and x-rays on your risk schedule. It is an active decision, not neglect.
When replacement is discussed
| Finding | Usually |
|---|---|
| Closed margin, healthy gum, no symptoms | Watch |
| Stained but closed margin | Watch |
| Open margin, decay on x-ray | Replace |
| Recurrent loosening | Investigate; often replace |
| Nerve symptoms | Root canal through the crown or replace, depending on the crown's condition |
| Chip exposing the core | Often replace |
| Appearance you dislike | Your choice, after the exam |
Questions to ask
What exactly did you find? Is the margin closed? Does the x-ray show anything? What would change your mind from watching to replacing?
Next step: Request a consultation or call (805) 565-5111. A request is not a confirmed appointment; the office confirms times during its listed hours (Monday to Thursday, 8 am to 5 pm, plus select Fridays).
