How dentists evaluate bone loss around teeth

Published October 20, 2026

In short

Bone loss is not seen directly in the mouth; it is inferred from several measurements taken together: probing depths and where the gum attaches relative to the enamel edge, bleeding on probing, tooth mobility and furcation involvement, and x-rays that show the bone level against the root. No single number diagnoses it; the pattern across the mouth does.

Each measurement in plain language

MeasurementWhat it readsWhat it cannot show
Probing depthHow deep the pocket between gum and tooth isBone directly; swollen gum deepens a pocket without bone loss
Attachment levelWhere the gum attaches relative to the enamel edge; the truer measure of support lost
Bleeding on probingActive inflammationPast loss that is now stable
MobilityLoss of support or bite overloadThe cause by itself
FurcationBone lost between the roots of a molar
X-rays (ADA)Bone height against the root, in two dimensionsEarly loss; the cheek and tongue sides

The National Institute of Dental and Craniofacial Research describes how periodontitis destroys bone and how it is assessed (NIDCR).

How findings are organized

The 2017 classification stages periodontitis by the worst site's attachment or bone loss and by tooth loss, and grades it by how fast it appears to be progressing and by risk factors such as smoking and diabetes (ADA). Stage and grade are planning tools, not labels on you.

Why the pattern matters

Generalized shallow loss, localized deep loss around a few teeth, and loss concentrated between molar roots each point to different causes and plans.

Questions to ask

Which teeth have lost support, and how much? Is it active (bleeding) or stable? What stage and grade? What would the plan be, and how will we know it is working?

The studio records periodontal measurements at examinations (studio).

Bone-support explainer and record

In words: a tooth is a post set in the ground. The probe measures how deep the trench around the post goes; the x-ray shows how much ground is left beside it.
