When saving a tooth may require an endodontist or periodontist
Saving a tooth sometimes needs procedures beyond general restorative dentistry. An endodontist treats the inside of the tooth: complex or repeat root canal treatment, root-end surgery, cracked and injured teeth. A periodontist treats the gum and bone that hold the tooth: deep pockets, surgery to regain lost support, crown lengthening so a tooth can be restored.
A referral usually means the plan is trying to keep the tooth, not that it is lost.
The two specialties
Both are recognized dental specialties with additional training. They work alongside the dentist who restores the tooth afterward: the specialist treats the root or the support, the restorative dentist rebuilds the crown of the tooth.
Scenarios
| Finding | Why a specialist may be involved | What that specialist commonly does | What happens next |
|---|---|---|---|
| A root canal that has failed or is complex | Curved or blocked canals; persistent infection | Retreatment or root-end surgery | Restorative dentist places the final restoration |
| A cracked tooth with pulp involvement | Diagnosis and treatment of the pulp (AAE) | Root canal; assessment of crack depth | Cusp coverage |
| Deep gum pockets or bone loss | Support must be stable before restoring (NIDCR) | Scaling and root planing; regenerative or pocket surgery (AAP) | Re-evaluation; then restoration |
| Decay or fracture below the gum | Not enough tooth above the gum to restore | Crown lengthening | Crown with a ferrule |
| Recession exposing a root | Root protection | Gum graft | Restoration if needed |
What changes the answer
How much sound tooth remains, bone support, extent of infection, a previous root canal, cracks, your health and medications, and your priorities.
How to use a referral well
Ask what question the specialist is answering, what the options are if the answer is unfavorable, and who coordinates the sequence. Bring x-rays and the restorative plan. Ask the specialist how their treatment affects the final restoration.
The studio works with specialists where a tooth-preserving plan needs them (studio).
Next step: Request a consultation or call (805) 565-5111. A request is not a confirmed appointment; the office confirms times during its listed hours (Monday to Thursday, 8 am to 5 pm, plus select Fridays).
