Why cavities between teeth can be hard to see
Decay between teeth starts on the contact surfaces just below where two teeth touch, a spot that a mirror, light and even a probe cannot reach, and enamel has no nerve supply, so these lesions usually cause no symptoms and show no visible hole until they are large. That is why they are mostly found on bitewing x-rays, where a lesion shows as a shadow in the enamel or dentin.
Why they hide
Two teeth touch at a tight point; plaque collects just below it where floss, not a brush, reaches. Enamel is translucent and nerveless; a lesion can hollow out dentin beneath an intact-looking surface. The Institute for Quality and Efficiency in Health Care describes how decay advances silently through enamel (IQWiG).
What changes how easily they are found
Crowding and overlap, existing fillings that cast shadows, how far the lesion has progressed, x-ray quality and angle, and your decay risk, which sets how often bitewings are taken; the American Dental Association's guidance ties frequency to individual risk (ADA).
What the dentist does
Visual check with a dry field, floss catching or fraying, transillumination with a bright light, and bitewing x-rays. MedlinePlus describes dental x-rays and what they show (MedlinePlus).
What the x-ray shows
| Shadow depth | Usual approach |
|---|---|
| Outer half of enamel | Monitor; fluoride; floss |
| Inner enamel to the junction | Monitor closely or treat, depending on activity and risk |
| Into dentin | Usually restore, kept small |
Symptoms, when they finally come
Sweet or cold sensitivity, floss shredding, a dark shadow seen through the enamel, or a chunk breaking away.
Prevention
Floss or interdental brushes daily (NIDCR), fluoride toothpaste, fewer sugar exposures, and bitewings on the schedule your dentist sets.
Next step: Request a consultation or call (805) 565-5111. A request is not a confirmed appointment; the office confirms times during its listed hours (Monday to Thursday, 8 am to 5 pm, plus select Fridays).
Between-teeth decay explainer
In words: two teeth meet like two pebbles touching. Decay starts in the shadow just below the touch point, where only floss goes.
| Tool | Finds between-teeth decay? |
|---|---|
| Mirror and light | Rarely, until large |
| Probe | Cannot reach |
| Floss | Catching is a clue |
| Transillumination | Sometimes |
| Bitewing x-ray | The main method |
Ask: how often do I need bitewings, given my risk?
