If your insurance denies a dental claim: questions for the plan and office
A denial is the plan's decision about what the contract pays, not a judgment that the treatment was unnecessary. Start with the explanation of benefits: find the denial reason or code, ask the plan in writing for the specific reason, the appeal steps and the deadline, and ask the dental office whether a narrative, x-rays, photographs or gum-measurement charting could support a corrected claim or an appeal.
Common denial groups
| Group | What it means | What may help |
|---|---|---|
| Frequency or age limit | Service already used this period, or outside age rules | Check the plan's limits; sometimes a different code applies |
| Waiting period | Service not yet covered since enrollment | Timing; little to appeal |
| Missing-tooth clause | Tooth lost before coverage began | Plan term; ask about exceptions |
| Alternate benefit | Plan pays toward a less costly option | You pay the difference; ask what was allowed |
| Annual maximum used | Plan paid its limit | Timing of further care |
| Not covered / cosmetic | Plan defines the service as excluded | Narrative and records showing clinical need |
| Documentation missing | X-rays, charting or narrative not received | Office resubmits with records |
| Coding error | Wrong code or tooth number | Office corrects and resubmits |
| Eligibility | Coverage not active on the date | Verify dates |
Questions for the plan
Exact reason and code; what documentation would change the decision; the appeal process and deadline; whether a second-level or external review exists.
Questions for the office
Was the claim complete; can you add a narrative, x-rays, photos or charting; will you resubmit or appeal; what is the deadline?
Keeping track
Dates of service, submission, denial and appeal; copies of everything; names of people you spoke with.

Next step: Request a consultation or call (805) 565-5111. A request is not a confirmed appointment; the office confirms times during its listed hours (Monday to Thursday, 8 am to 5 pm, plus select Fridays).
