A dental preauthorization is not always a payment guarantee

Published October 20, 2026

In short

Not necessarily. A preauthorization or predetermination is the plan's advance review of a proposed treatment, and it usually states whether the procedure is a covered benefit and an estimated payment. It is not a promise to pay, because the final claim is judged against eligibility on the date of service, the remaining annual maximum, deductibles already applied, frequency limits, coordination with another plan, and whether the treatment actually performed matches what was submitted.

The vocabulary

  • Pre-treatment estimate: the office's estimate of your share, based on what it knows of your plan
  • Predetermination: the plan's written estimate after reviewing the proposed treatment and records
  • Preauthorization: some plans require advance approval for certain procedures before they will consider paying; it still carries the conditions above

Why an estimate can differ from the final payment

ReasonExample
Eligibility changedCoverage ended or changed before the visit
Maximum used upOther claims paid in the meantime
Deductible applied elsewhereMet by another claim first
Frequency limit reachedA service already claimed this year
Different procedure performedThe dentist found more, or less, once treatment began
Another plan involvedCoordination of benefits changes the split
Time limit on the estimateEstimates expire

How to use a predetermination well

Ask for one for any major treatment; read the conditions; ask the plan how much of the maximum remains on the date of service; schedule within the estimate's validity; tell the office if your coverage changes.

Questions to ask

Plan: is this an estimate or an authorization; what could change the amount; when does it expire? Office: which codes were submitted; what happens if the treatment changes once started; will you re-submit?

This page does not state fees or interpret any plan. The studio provides written treatment plans with codes for submission (studio).

Next step: Request a consultation or call (805) 565-5111. A request is not a confirmed appointment; the office confirms times during its listed hours (Monday to Thursday, 8 am to 5 pm, plus select Fridays).

Predetermination checklist

  • Predetermination requested for major treatment
  • Conditions read; expiry date noted: ________
  • Remaining annual maximum confirmed for the date of service
  • Deductible status confirmed
  • Frequency limits checked
  • Other plan (coordination) disclosed
  • Office knows to re-submit if treatment changes

Estimated share (from the plan, not a promise): ________
