What are refinement aligners?

Published October 20, 2026

In short

Refinement aligners are an additional series of trays made from a new scan after the first series ends, to finish movements that did not fully happen. Studies comparing planned with achieved tooth positions show the first series rarely reaches the plan completely, so refinements are a normal stage in many cases, not a sign of failure.

Why teeth do not always follow the plan

Some movements are predictable (tipping); others less so (rotations of rounded teeth like canines and premolars, extrusion, intrusion, moving roots). Wear time, lost attachments, and individual biology all vary. The plan on the screen is a prediction; the mouth is the result.

What happens at a refinement

  1. A new scan of where the teeth are now
  2. Review of what is left to achieve, for the smile and for the bite
  3. Sometimes new or repositioned attachments
  4. A new, usually shorter series of trays
  5. Retainers only once the result is accepted

What to ask

What is the goal of this refinement: finishing the smile, settling the bite, or both? How many trays? Is this round included in my plan, and how many rounds are? What would make a second refinement likely? Could braces finish this faster for a particular tooth?

When refinement is not the answer

If a tooth has not moved at all after full wear, the cause is checked: an attachment missing, a tooth fused to bone, or a movement aligners cannot do well. Then the dentist may suggest a different approach or a referral. The American Dental Association notes that treatment choices depend on the individual case (ADA).

Patience and wear

Refinements work the same way as the first series: full wear hours, tray changes as directed, and attention to fit.
